PURPOSE: The purpose of this annual report is to provide the Department of Education (ED) with data required by Title I, Part A, of the Elementary and Secondary Education Act (ESEA), as amended by Public Law 103-382, for use in computing formula allocations to local educational agencies (LEAs).

REQUIREMENTS: The information in this report must be submitted annually by all State educational agencies (SEAs) in order for ED to establish a prior year base on which to determine Ahold-harmless@ guarantees for each LEA when computing Title I, Part A allocations. No sampling or estimating may be used in preparing this report. Since these data are a critical element in the statutory Title I formula used to distribute Federal funds, they are subject to audit and must be supportable from documented records.

ALLOCATION OF TITLE I, PART A FUNDS TO LEAs

1. LEA code - Identify the LEA code. This information is required for data processing purposes. A list showing the LEA code and the names of the LEAs for the State is attached. Please use the codes provided. ED is making available electronic copies of the LEA codes and names for your State.

2. Allocation data - Enter separately the allocation amount for school year 1998-1999 of each LEA and the amount each State retained for Part D, Subpart 2 purposes for (1) Basic Grants, authorized in Title I, Part A, section 1124 of ESEA, (2) Concentration Grants, authorized in section 1124A of Title I, and (3) Targeted Assistance Grants, authorized in section 1125 of Title I. Please list all of the LEAs in your State, furnish a total amount for each LEA, and provide a grand total for the State. If the State educational agency (SEA) allocates no Title I, Part A funds to an LEA because it is ineligible, show the amount as zero. Include allocations for all LEAs entitled to receive a Title I Basic Grant, Concentration Grant, or Targeted Grant allocation even if they chose not to participate. Do not include carryover, reallocation, or Title I, Part D, Subpart 2 funds in the LEA allocation data submitted. In cases where there is only one LEA for the State, the SEA should enter the Title I, Part A amounts for the LEA after it has reserved funds required for Title I State Administration and School Improvement as provided in section 1603(c) and 1003 of ESEA.

   Note, if the SEA allocates funds to LEAs that are not on the list provided by ED, please be sure to include those LEAs and the Title I, Part A amounts allocated to them for SY 1998-1999 when submitting the information required by this survey form to ED.

SECTION B - CERTIFICATION BY STATE EDUCATIONAL AGENCY

Complete the certification portion of the report. The appropriate official in the State educational agency (SEA) must sign this report to certify that the information submitted is complete and accurate. The information provided is subject to audit, and the SEA should maintain documentation.
