| Comments are invited on a revision to an information collection. It is for the Child Care Provider Loan Forgiveness Application and the Child Care Provider Loan Forgiveness Forbearance Form. The Forgiveness Application is used to determine whether borrowers meet the eligibility requirements for Child Care Provider Loan Forgiveness Program. The Forgiveness Forbearance Form is required to fulfill program guidance that provides forbearance for child care providers. Comments are due on or before June 6, 2005. |

If a school fails to make a required refund of a Federal Family Education Loan Program or William D. Ford Federal Direct Loan Program loan, a borrower uses this form to apply for a discharge of the portion of the loan that was not refunded. Comments are due on or before November 26, 2004. |
