Please be advised that the Financial Status Report (FSR) for grants awarded under the Carl D. Perkins Vocational and Applied Technology Education Act of 1990 (Perkins II) has been amended and approved by the U.S. Office of Management and Budget (OMB Number 1830-0519, Expiring March 31, 2001). Please note that previous versions of the form have expired and cannot be used.
The FSR is used by grantees to file their annual financial reports with the Division of Vocational-Technical Education (DVTE) as provided in the Education General Administrative Regulations at 34 CFR 80.41. The forms and instructions can be downloaded at the following URL:

In accordance with 34 CFR 80.41(b)(4), FSRs are due 90 days after the end of the grant year (federal funding period), as specified in Block 6 of the Grant Award Notification for each respective grant. The following is a schedule of reports that are to be completed by December 31, 1999:

Please be aware of the correlation between the liquidation of grant obligations and submission of a final report. The regulations found in 34 CFR 80.23 state:
A grantee must liquidate all obligations incurred under the award not later than 90 days after the end of the funding period (or as specified in a program regulation) to coincide with the submission of the annual Financial Status Report (SF-269). The Federal agency may extend this deadline at the request of the grantee.
Requests to extend the deadline to submit a final FSR for reasons other than needing to extend the liquidation period will be granted upon receiving a request detailing the circumstances surrounding the delay. Such requests must be filed in writing before the December 31 due date of the report to be considered.
